Mission
Community Health & Wellness (CHWC) is committed to keeping communities healthy. In a compassionate, high-quality and patient-centered environment, services are accessible and provided without discrimination, with cultural competence, and where all people matter regardless of ability to pay. Through coalitions, advocacy and sound fiscal management, these services will be sustained over time.

Our Vision
CHWC will offer and continue to develop a range of healthcare services for the underserved throughout northwest Connecticut. It will achieve the reputation of provider of choice for affordable, high quality services that achieve positive patient outcomes. A welcoming and caring environment will be experienced by all patients and their families.

Elizabeth Caruthers is a Registered Dietitian, board-certified Diabetes Care and Education Specialist and Coordinator for the Nutrition and Diabetes Programs at Community Health and Wellness Center. She received her BS in Nutrition from the University of Maryland in 1995. Elizabeth’s background includes acute care, private practice, and community health.



She works with both children and adults and loves helping her clients experience a better quality of life by making healthy changes step by step. Elizabeth has been a student of meditation for 15 years and has found mindfulness to be the most valuable tool in her work and life

BETTY the Medical Mobile Unit will be at he following locations:
Tuesdays at Sharon Hospital 9 AM-12 PM
Wednesdays at The Union, Cornwall from 9 AM-12 PM Wednesday June 22 No Service
Thursday June 30, July 14, 28 August 11, 25 9AM-11AM 
Thursdays at Salisbury VNA 9 AM- 11 AM and 
Pilgrim House, North Canaan 12:15 PM – 2:15 PM
Fridays at Community Soup Kitchen, Torrington
from 10:30 AM–1 PM
